BPJS Kesehatan Receives Additional Rp20 Trillion in Funding

RRI.CO.ID, Jakarta - The Indonesian government is providing an additional Rp20 trillion in funding to the Health Social Security Agency (BPJS Kesehatan) to cover the cash flow gap between participants' contributions and benefit payments. The funds will be disbursed in two stages, with Rp10 trillion in October and another Rp10 trillion in November 2026.
Health Minister, Budi Gunadi Sadikin said the disbursement had been approved by the Ministry of Finance about two weeks ago. The additional funding is needed to maintain BPJS Kesehatan's cash flow and ensure that claims to hospitals can be paid smoothly.
BPJS Kesehatan President Director, Prihati Pujowaskito explained that the additional funding is needed because the contributions collected remain lower than the benefits that need to be paid.
As of September 2026, BPJS Kesehatan had collected Rp118 trillion in contributions, while benefit payments had reached Rp132 trillion. This resulted in a cash flow gap of around Rp14 trillion that needs to be covered to ensure that services for participants can continue to run properly.
“We would like to thank the President, the government, and the Minister for providing Rp20 trillion in additional funding,” Prihati said.
Prihati said one of the factors affecting BPJS Kesehatan's cash flow is participants' compliance with contribution payments. Currently, around 52 million participants are recorded as inactive and are not paying their contributions, which also affects BPJS Kesehatan's revenue.
In addition, BPJS Kesehatan contribution rates have not been adjusted for the past six years. At the same time, medical service costs have been affected by inflation, while the utilization of hospital services has increased.
To address these challenges, BPJS Kesehatan plans to shift its service system from a volume-based approach to a value-based approach. The change is expected to improve the delivery of appropriate services while also supporting improvements in BPJS Kesehatan's cash flow.
“Going forward, we will implement a value-based approach. This is also part of our commitment to improving the quality of services so that they better meet participants' needs and are more appropriately targeted,” he explained.
